A Professional Practice Gap is defined as the difference between health care processes or outcomes observed in practice, and those potentially achievable on the basis of current professional knowledge.


Presentations are required in advance to allow the planning committee to review each presentation, and to ensure content is appropriate for CME credit. The committee may also provide feedback to help you improve your presentation, and to make sure learning objectives outlined for the presentation were addressed.


Presentations can be sent via email to srbelafsky@ucdavis.edu. You can also submit them online through a shared Berkeley Box folder. You should have received an invitation from noreply@box.com. Email mmeyer@berkeley.edu if you need a new invitation.

Upon accepting the offer to present / instruct at a COEH Continuing Education event, you will receive a resource packet and a link to a Qualtrics form. This form is where you will provide your:

  • Contact information

  • Presentation details

  • Short biography

  • Current resume / CV (via upload)

  • Professional Headshot (via upload)

  • Distribution Permissions

When submitting your headshot, please provide a photo that is at least 512px by 512px. This photo will be hosted on our registration website, next to your name and biography. We may also use this photo to help promote the course through publishing ‘Speaker Features,’ and highlighting your presentation. If you do not want your photo to be used to promote the course, you do not need to upload it to the Speaker Information Form.


COEH Continuing Education provides honorarium as payment granted to an individual in recognition of participation at COEH Continuing Education events. The standard honorarium for speakers is $100/hour of instruction, assuming the speaker’s visa and tax status allows the acceptance of honorarium. Honorarium payments are rounded up to the nearest half hour. University of California staff and students are not eligible to receive honorarium payments.

If you plan to receive honorarium or travel reimbursements from UC Berkeley COEH Continuing Education, you must complete the following forms:

The W9 and Conflict of Interest form must be faxed together to (510) 664-7209. This is a secure fax line specifically designed to receive these types of forms, and to enter new vendors into the Berkeley payment system. Please contact us with any questions. Honorarium payments are requested by COEH-CE, and are subject to final campus approval. 

Note: If you must email us the W-9 to submit on your behalf, please omit your SSN on the form and call us at (510) 642-8365 to provide this information by phone.



If you wish to request travel accommodation and/or reimbursement, please consult COEH CE staff in advance. Only pre-approved travel expenses will be eligible for reimbursement.

Airfare, hotel, taxi, shuttle, mileage, rental car, parking, and meals may be reimbursed according to University of California Office of the President Policy G-28, available online at: http://policy.ucop.edu/doc/3420365/BFB-G-28.


COEH CE will provide additional travel policy information and instructions as necessary.


Course Materials

In the effort to go paperless, we request final presentations be submitted no later than one week before the course. Learning materials are saved as watermarked PDFs (two-slides per page), and shared with learners online. This allows learners to download or print materials in advance to better accommodate different learning styles and needs. You are not required to share your presentation with learners online. Alternatively, you can elect to submit a redacted version for distribution.

Receiving materials in advance also allows us to ensure the presentation works as anticipated on conference laptops, projectors, etc. and to test embedded videos, audio, and animations.

If you would like us to print worksheets or similar materials, please provide them no later than 2 weeks in advance of the course. Speakers will be responsible for printing any required materials after the due date.
